Pharmacodynamics (Mode of Action)
Bactericidal activity of cefpodoxime is by inhibition of cell wall synthesis
- Bacterial cell wall is composed of peptidoglycan (polysaccharides + protein) and functions as a protective casing for the bacterial cell.
- Cefpodoxime inhibits the transpeptidase enzyme, required for linking of peptidoglycans.
- Thus, prevents the formation of peptide cross-links, thus the bacterial cell wall is weakened and gets killed.
Antimicrobial Spectrum of Cephalosporins
First generation |
Cefadroxil Cephalexin Cephradine |
Streptococci except for penicillin resistant strains, Staphylococcus aureus. |
Second generation |
Cefaclor Cefprozil Cefuroxime axetil |
E Coli, Klebsiella, Proteus, haemophilus influenzae, moraxella catarrhalis. Not as active against gram positive organisms as first generation agents. |
Third generation |
Cefdinir Cefixime Cefpodoxime Ceftibuten Ceftriaxone |
Expanded coverage of gram-negative organisms enhanced coverage of Protein edgars and Providencia species. Extended activity for staph aureus, streptococcus pneumoniae and streptococcus pyogenes comparable to first generation agents. |
Bacterial Spectrum of Ceflino: Cefpodoxime proxetil demonstrates a potent inhibitory activity against the common respiratory pathogens including
- Beta lactamase producing strains of M. catarrhalis
- H. influenzae
Cefpodoxime is highly active against penicillin- susceptible S. pneumoniae.
Cefpodoxime has enhanced antistaphylococcal activity which distinguishes it from other orally active third generation cephalosporins such as cefixime.
Pharmacokinetics
- Metabolism: Esterified to active metabolite
- Elimination: 29-33% unchanged thru’ urine
- Protein Binding: 21-29%
- Half Life: 12 hours
Cefpodoxime is More efficacious than Cefixime because…
- Cefpodoxime offers two fold higher tissue concentration
- Cefpodoxime offers more than double Area Under Curve for Tissue
- Cefpodoxime is superior in coverage against
- catarrhalis
- influenza
- pneumoniae
Cefpodoxime has the lowest MIC against common pathogens involved in respiratory Infection as compared to Cefixime.
Unique Selling Points (USPS)
- Extended spectrum oral antibiotic agent.
- Twice daily administration and safety profile increases compliance and decreases antibiotic failure rate.
- In areas where common respiratory pathogens show decreased sensitivity to penicillins and macrolides, cefpodoxime can be used as empirical (based on Experience) first line therapy in respiratory tract infections.
- Being used as a step down (from Injectable to oral) from parenteral cephalosporin.
- Ceflino is available in orange flavor – excellent taste.
